Bella’s House of Doodles Membership Agreement

 

This Membership Agreement (the Agreement) is effective  _____________________ by and between Bella’s House of Doodles, LLC,  hereinafter referred to as “The Company”, accessed via 507-257-2011 with a business address of 107 598th Avenue, Eagle Lake, MN 56024, and all future Clients of “The Company” (Bella’s House of Doodles, LLC) hereinafter referred to as “Client”.

 

By accessing, browsing or using the “Company” (Bella’s House of Doodles, LLC) website or by selecting I accept during the membership registration, you represent that you have read, understand, and agree to be bound by the terms and condition of this membership Agreement hereinafter referred to as “Agreement”. 

 

1. Nature of the Service

Based on the selected membership the “Company” will provide these services:

 

                       Bella’s Basic Membership:             ⃞

-No Holiday Up-charge for Boarding Reservations

-$1 off Boarding Each Boarding Reservation (With the exception of Holidays)

-5 Days of Daycare Monthly

-1 Nail Trim or Dog Wash* Monthly (Self Serve Dog Wash*)

-Includes 1 FREE Weekend of Boarding Annually (2 Nights)

 

The fee for this membership is $120 due monthly. This contract duration is 12 months. Client may upgrade for a Premium Membership at any time. Upgrades result in a new contract.

                   Bella’s Premium Membership:              ⃞

• Priority Boarding for Holidays

• No Holiday Up-Charge 

• 1 off Boarding Each Visit (With the exception of Holidays)

• 10 Days of Daycare Monthly

• 1 Nail Trim or Dog Wash Monthly

• Includes 2 FREE Weekends of Boarding Annually (2 nights each reservation)

 

The fee for this membership is $220 due monthly. This contract duration is 12 months.

                                            Bella’s Bubble Club:                            ⃞

• Unlimited Bath & Blow-Dry services 

• No Up-Charge for Appointment Add-ons​

• Free Samples of Espana Silk Products (Upon Registration)

The fee for this membership is based on Pet’s weight and is due monthly:

Toy (0-10lbs) - $40      ▢

Small (11-20lbs) - $45    ▢

Medium (21-45lbs)- $55   ▢

Large (46-85lbs)- $65   ▢

Client may choose a monthly or annual contract for this membership.

Clients who select a monthly contract may cancel at any time with no consequences.

Clients who select an annual contract will receive 10% off all grooming services provided by the “Company”.

Regardless of contract selection the Client will be billed monthly for this membership.

Appointment Add-ons are only applicable to Standard Grooms, Baths & Brushes, and Baths & Blow-outs. Appointment Add-Ons include gland expression, teeth brushing, de-shedding, and de-matting. Clients may schedule a solo service with their Bubble Club Bath & Blow-Dry, but will be charged the price of that service. See our grooming page for pricing.

1. User Registration and Information

Client shall fill in the correct information requested in this form. Client shall be required to create an account on Paw Partner. Client shall create a username and password during the account creation process. Client shall be required to promptly update User information on Paw Partner. Client shall be responsible for: a) all use of Paw Partner made by Client’s username and password, and b) maintaining confidentiality of the Client’s username and password.

 

1. Privacy 

The “Company” shall not sell or rent the Client’s personal information to third parties without Client’s explicit consent. The “Company” shall store and process the Client’s information on computers located in the United States. The “Company” shall be permitted to access and modify Client’s information. 

 

1. Indemnity 

Client shall indemnify and hold the “Company” (and it’s owners, managers, and employees) harmless from any claim or demand, including reasonable attorneys’ fees, made by any third party due to arising out of Client’s breach of the “Agreement”, or Client’s violation of any law or rights of a third party. 

 

1. No Agency

No agency, partnership, joint venture, employee-employer or franchiser-franchise relationship is intended or created by this “Agreement”.

 

1. Terminating Membership

Clients may choose to buy-out their membership at 6 months for 50% of the remaining balance on their contract. Buy-outs prior to 6 months will owe 75% of remaining balance on account. Upon buy-out Client terminates this “Agreement” and no longer has access to the services provided by this “Agreement”. This section does not apply to Bella’s Bubble Club Monthly Contracts.

 

1. Governing Law

This “Agreement” shall be constructed under and governed in accordance with the laws of the state of Minnesota.

 

1. Arbitration

Any dispute arising under this contract shall be resolved under the commercial arbitration rules of the American Arbitration Association.

 

1. Limitation of Liability 

Client shall not hold the “Company” responsible for other Client content, actions, or inactions.
